Thame Community Hospital hold open day for new 'community hubs'

Thame Community Hospital is today (13/09) giving people the chance to find out more about their new 'community hubs'.

The Bucks NHS Trust is currently piloting new ways of providing care, to make it easier for people to get the right support.

The aim is to provide more of a one stop shop for patients so they can have improved care and don't have to travel around to access different services.

The open day takes place at the hospital in East Street on Wednesday 13 September from 2-4.30pm.

As well as an opportunity to find out more about the new hub there will also be representatives from local partner organisations such as Age UK Oxfordshire, Carers Bucks and Prevention Matters.

Free refreshments will also be available.
